How to choose a healthcare marketing agency: 12 questions to ask

Most doctors and hospital leaders have a story about a marketing agency that promised a flood of patients and delivered a pile of reports instead. Healthcare marketing is different from marketing a restaurant or a shop. Patients research carefully, medical advertising rules are strict, and a single misleading claim can damage a reputation built over decades.

Whether you are a surgeon in private practice, a hospital administrator or a health tech founder, these 12 questions will help you separate agencies that understand healthcare from those that simply list it on their website.

Experience and focus

1. What share of your clients are in healthcare? An agency that works across many industries may be excellent, but it will learn healthcare on your time. Ask how many current healthcare clients they have and which specialties.

2. Can I see case studies in my specialty or a similar one? Look for specifics: what the starting point was, what they did, and what changed, with figures you can understand. Be cautious of vague claims like “we grew their business” with no detail.

3. Can I speak to a current client? A confident agency will connect you with one. Ask that client what the agency did well and what they would change.

Strategy before tactics

4. How do you decide what to do first? A good answer starts with your situation: your market, your competitors, the procedures or services that matter most to you, and where patients currently find you. Be wary of an agency that recommends the same package before understanding your practice. See how we work for the approach we follow.

5. Which patients or customers are you trying to bring me, and how will you measure that? “More traffic” is not a goal. Booked consultations, new patient calls, admissions for a specific service line, or qualified demo requests for a health tech product are goals.

Compliance and ethics

6. How do you handle medical advertising rules in my country? In India, the National Medical Commission governs how doctors may advertise. In the US, patient privacy law (HIPAA) and state medical board rules apply. In the UK, the GMC and the ASA set the rules. Ask how the agency checks content before it goes live. Our approach is on our compliance page.

7. How do you get reviews? The right answer involves asking real patients at the right moment. Fake reviews, paid reviews and only asking happy patients (“review gating”) break platform rules and can get your profile penalized.

8. Who owns the accounts and the content? Your website, Google Business Profile, ad accounts, social media pages and analytics should be in your name, with the agency given access. If you part ways, you should keep everything.

Execution and reporting

9. Who will actually work on my account? Find out whether the senior people you meet in the sales call will be involved, or whether work is handed to a junior team or subcontracted.

10. What will your monthly report show? Ask to see a sample. A useful report connects activity to outcomes: calls, bookings, enquiries and cost per new patient, not just likes and impressions.

11. How long before I see results, honestly? Paid ads can bring enquiries within weeks. SEO and personal branding usually take several months to build momentum. An agency that promises page one rankings in a few weeks for competitive terms is either guessing or cutting corners.

Contracts and cost

12. What does the contract commit me to? Understand the minimum term, notice period, what is included each month, and what costs extra, such as ad spend, video shoots or software. Clear scope protects both sides.

Red flags

Walk away, or at least ask harder questions, if an agency:

  • Guarantees rankings or a specific number of patients
  • Offers to buy reviews or followers
  • Keeps your accounts in its own name
  • Cannot explain what it will do in your first 90 days
  • Shows no healthcare work at all

A simple way to compare agencies

Question Agency A Agency B Agency C
Healthcare focus and relevant case studies
Clear first 90-day plan
Compliance process
You own all accounts
Sample report shows outcomes
Contract terms are clear
